Proviso Township Mental Health Commission (PTMHC): TASC Care Coordination Program

Since November 2011, TASC has partnered with the Proviso Township Mental Health Commission (PTMHC) to institute a collaborative care delivery model in Proviso Township. In Proviso Township’s Network of Care, TASC care coordination services are helping to improve the overall delivery of services, improve the health of the residents affected by mental health and/or co-occurring disorders, and reduce healthcare costs for the State and designated agencies.

A recent collaboration between the PTMHC and Loyola Hospital’s ACE (Accountable Care Entity) Medicaid program has resulted in expanded TASC care coordination services to ACE clients in need of treatment for mental health and/or substance use disorders. TASC currently provides care coordination services for the PTMHC Network of Care and the Loyola ACE collaborative to advance TASC’S vision of providing quality care to the underserved populations. Care Coordinators serve as provider liaisons for clients, as resources and information portals for individuals and the community, and as central information coordinators for the PTMHC.



Proviso Township encompasses the villages of Bellwood Berkeley, Broadview, Brookfield, Forest Park, Hillside, LaGrange Park, Maywood, Melrose Park, Northlake, North Riverside, Stone Park, Westchester, and Western Springs. TASC’s Care Coordination programming is delivered to verified residents of Proviso Township.

TASC’s Role

TASC Care Coordinators educate providers on clients and their individual and family services needs, and provides information on insurance opportunities and client benefits under health reform. We conduct one-on-one client sessions, based on individual needs, to provide linkages to community-based services, focusing not only on critical mental health, substance use, medical, and 12-step/peer support group needs, but also on managing linkages to a full continuum of services e.g., vocational/educational, housing, benefits assistance, senior-specific, youth after-school and prevention, developmental disability, domestic violence support, emergency homeless support and food pantries.

Benchmarks of TASC’s Care Coordination program include increased: utilization of services provided; clients’ commitment to engaging in treatment; recognition within the community; community referrals; relationships with Proviso stakeholders; and resource network development.
